What will I need for my stay?

In addition to toiletries we encourage all of our patients to bring at least 2 weeks worth of clothing. It is an important part of therapy to get you into your own clothes and functioning as you would at home or in your community.

What happens in my therapy sessions?

You will take an active part in therapy in 30 to 45 minute sessions throughout the day for a total of 3 hours at least 5 days a week. Daily progress needs to be demonstrated and participation in therapy is a requirement.

Can my family stay with me?

We understand that family plays a crucial role in the recovery process. Please speak with your clinical liaison or case manager about overnight guest. We make accommodations on an individual basis.

How will my regular doctors know what happens at REHAB?

Your REHAB Physician will dictate a discharge summary that can be sent to your primary doctors upon request.

What is the discharge process?

Our social work/case managers will begin the discharge process from the time of admission as the earlier this is started, the smoother the transition will be. The discharge plan may change during your stay depending on your progress and medical needs, but rest assured we will communicate any necessary changes. Together with your interdisciplinary team, the case managers will ensure you have the needed prescriptions, medical equipment, follow-up appointments with Primary Care Physician (PCP) and Medical Specialists, Home Healthcare services or Outpatient Therapy confirmed prior to discharge. The case managers will also help you transition to another level of care if your interdisciplinary team at Acute Rehabilitation Hospital recommends so.

How do I get my medicine after I leave REHAB?

Prescriptions will be sent electronically to your chosen pharmacy.
